Help us improve our forms
We are pleased to announce that we are revising several of our forms this year. As valued members, we invite you to share your feedback on what works well, what needs improvement, and what is missing in Form C and Form F. We are also looking for local authorities and independent fostering providers to test and trial the revised Form C and Form F.
What works well: We recognise that our current Form C and Form F have aspects that work well, and we want to build upon them. Your input on what works well will help us retain and develop those strengths. Whether it's the layout, clarity of instructions, or ease of data entry, your feedback will contribute to maintaining a user-friendly experience.
What needs improvement: We acknowledge that there is always room for improvement. Your insights on confusing sections, duplication, or unclear instructions will help us enhance usability and efficiency. By addressing these ‘pain points’, we aim to create more intuitive and user-friendly Form C and Form F that meet your needs and expectations, but also improve the experience of being assessed for our families and carers.

Testing and trialing the forms: To ensure the quality of Form C and Form F, we invite local authorities and independent fostering providers to participate in testing and trialing.
