CoramBAAF publish form AH in 2 different formats: Word and PDF.

Forms AH/AH2 had a major revision with sector wide consultation in 2021/2022. The forms had minor amendments in 2025.

The 2025 revisions were as follows:

Section A

Added additional highlighting to section that identifies where forms should be returned to when completed

Section B

B1. Applicant to supply NHS number ( most people now have easy access  to this information with developments such as the NHS app)

B1. Question added about applicants living outside of UK

B2. Medication question expanded to ask about medications sourced online

B4. Questions re alcohol use revised

B5. Consent section adjusted so there is permission from applicant for the medical adviser to contact all health professionals for further information

Section C

C1. GP "safeguarding " question expanded to include a more specific prompt re problematic drug and alcohol use.

C1. Question re gaps in GP records refined to ensure clearer detail is recorded

C3. Mental health question slightly expanded to ask for more detail of specific episodes of ill health

C4. Extra detail on request to include hospital letters C6. Question re impact on health split into more specific areas to encourage clearer information to be supplied Extra date box on signature section on PDF

Use of Form AH in Practice

Most areas now use electronic record systems. As previously, the Word version can be transferred, completed and stored electronically. It is also possible for individual organisations to format form AH for use as a template within their EPR.

Some organisations may find that a PDF version is easier to use. Advantages include being able to sign the document digitally.

CoramBAAF would recommend that medical advisers, local authorities, agencies and GP services liaise locally to agree which format works best with local systems.
